Do collagen supplements work?

Collagen supplements are among the best-selling products in the beauty sector. Powders, shots, capsules, drinkable ampoules — the promise is everywhere the same: firmer skin, fewer wrinkles, more elasticity. This review works through that promise using the fixed structure of this section.

What actually happens to ingested collagen
Collagen is a protein. Proteins are not absorbed whole in the digestive tract; enzymes break them down into amino acids and smaller peptides. These enter the bloodstream and become available to the body as building blocks — for whichever tissue currently needs them.
That already refutes the most intuitive assumption: collagen you drink does not travel to the skin as collagen and get incorporated there. At this point the body does not distinguish between collagen from a supplement and protein from an ordinary meal.
There is, however, a more nuanced hypothesis worth taking seriously: certain collagen-specific peptides might partly survive digestion and act in connective tissue as a signal that stimulates the body's own collagen production. This hypothesis is plausible and the subject of current research — but it is not the same as the direct incorporation implied by advertising.
What the studies show
Several randomised controlled trials as well as systematic reviews and meta-analyses exist. They predominantly report small but statistically measurable improvements in skin hydration and elasticity [1][2].
The methodological limitations are considerable, however, and are named in the reviews themselves:
- Industry funding: a strikingly high proportion of the studies were funded by companies that sell the product under investigation.
- Small participant numbers: many studies involve well under a hundred people.
- Short durations: eight to twelve weeks is typical. Little is known about the effect after a year.
- Inconsistent measurement: skin hydration and elasticity are recorded with different devices and definitions, which makes comparison difficult.
- Heterogeneous preparations: source, molecular size and dosage differ considerably between studies.
There is a further point that is rarely discussed: some of the reported effects may simply arise from participants taking in additional protein. In studies with a protein control group, the differences tend to be smaller.
Realistic benefit
- Possibly a small improvement in skin hydration and elasticity
- Very good safety profile — serious adverse effects are rarely reported
- As an additional protein source where intake is low, generally unproblematic
Risks and drawbacks
- Cost: substantial ongoing expenditure for an effect that is small at best. Over a year, typical preparations quickly add up to several hundred euros.
- Product quality: food supplements are not subject to the approval requirements applied to medicines. Composition and labelling vary.
- Allergies: allergic reactions are possible with preparations from fish or bovine sources.
- Displacement effect: the most practically relevant point — attention and budget flow into a supplement instead of measures with considerably better evidence.
Assessment
The decisive question is not whether collagen supplements can have any effect at all. It is whether that effect justifies the outlay — compared with the alternatives.
Consistent sun protection and a topical retinoid have considerably better evidence at lower cost. Anyone already doing those things can additionally try a supplement without risking anything. Anyone not doing them is investing in the wrong place.
A small effect is possible but not reliably established — and considerably smaller than advertising suggests.

What this means in practice
Anyone wanting to try a preparation should stay realistic: a visible difference in the mirror is unlikely. Measurable changes in studies do not automatically translate into perceptible changes in daily life. And a preparation that makes no difference after eight weeks will hardly make one after eight months.
